Thank you for your interest in providing goods or services to A-C-T Environmental & Infrastructure, Inc.
Please complete the application below and submit it with the required documentation to be set up in our system.
This must be completed before we can issue a purchase order and/or payment for goods and/or services provided. Please provide proof of insurance with your company name identified as the insured. A-C-T Environmental & Infrastructure, Inc., 1875 West Main Street, Bartow, FL 33830, must be a certificate holder and listed as an additional insured.

A-C-T's default vendor payment term is Net 60, unless specifically indicated differently in individual subcontractor, work authorization, or field services agreements.
